Truss Rod Nut Backed Out

I’ve got an LTD B-50 that I bough new forever ago. I was trying to set it up, but I wasn’t getting any response from adjusting the nut, it was turning pretty freely. It backed completely off of the truss rod.

The longer barrel nut comes out of the headstock and I can see a second nut and a wedge piece on the underside of the fretboard. I’m not sure what to do from here. I was considering lock-tite, but figured I’d see what people recommend here first. Any recommendations on how to go about disassembling/reassembling everything so that I can adjust the truss rod? I know this has a two way truss rod, so I assume that this may be a little more complicated.

If it has backed off the rod, it's probably not a two-way truss rod.

If the neck is so back bent that you are loosening the truss-rod that much, it may be a lost cause.
